My form is broken on mobile

My form at https://www.bethin.co/start is broken on mobile. I am using an iPhone 13 mini. My website is using squarespace. The form works in preview and it looks like it works below but not on my website on an actual mobile phone. I am embedding it with the following link

    I did not have a chance to go through your form in depth....but could you ensure that both the container div is set to 100% of the screen width and height
