Hi,
I want a 1-5 option scale and one more option for “question not relevant”. Since I can not add labels where I want it is hard to describe it with labels. Any ideas how to solve it?

We don’t have this option to add more labels to the opinion scale, but you could add a yes/no question (or another variation of this) afterwards stating if it doesn’t apply to them, to click that option. It’s not the best workaround, but it’s something!
