How can I add a disclosure to all forms?

How can I add a simple disclosure to all my forms? 

For compliance purposes, if I want to have automated texting, I need to add a disclosure statement and/or link to a disclosure statement.

    @RickBosl - you can use a Statement page to put in the disclosure. if you want to have the user ‘acknowledge’ the disclosure before continuing, then you could adapt a Legal page
