Is it possible to creat a form where my followers are able to fill in their name and email and when they hit submit it gets sent to an email that I assign, like a senator for example.

yes you can, it is am important feature. go to your form settings, then type on notification and then on the field sent a notification to add the email you want the form to be set to and press save. ce tout
