Issues with Salesforce Integration

I am getting this error when I try to activate Form Field Mapping in Salesforce. 

Cannot Activate Mapping

Can't Map Same Salesforce Field To Two Questions: undefined

I tried multiple scenarios but it didn’t allow to activate mapping even if one single field is mapped. 
For some of our forms integration works correctly, but for some of them it doesn’t allow to activate mapping. 

Issue is the same as it mentioned here -

    Hi @Liz,

    Only one field Email is mapped on the form, but it doesn’t allow to activate the form.
