Creating a custom end screen/message?

Hey community - 

My team is wanting to have users fill out a form to display a customized card. This “card” would have a personal message and a recipients name. I was hoping that there would be a way to use dynamic content to repeat this info the users puts in on the end screen. 

If anyone has insight on how this might be possible, it would be super helpful! Thanks!
