I created my first form with 14 questions, however it was generated with only 12 questions and cut off my last two questions. Why? It won't let me add the last two?
Also, how do I add multiple choice answers on the form?

It’s possible that there’s a logic jump skipping over the questions, but I can confirm this once I see a few screenshots. 😀
As for adding multiple choice answers, you can type them in like this:
Life is a series of awkward moments separated by snacks.
