Can I turn the test into a Typeform questionnaire that ads up the score, gives the user the results and sends me their score with their contact information?

You can capture the scores associated for each answer and connect your Typeform up with a platform like Google Sheets to store the data ( including scores and contact information)
On GoogleSheets it should then be relatively straightforward to setup the report in the manner you are wanting.
