Create a tabular form with multiple options to answer

I want to design a form which looks like table in Excel and in that form answers can be multiple like yes or no, Numbers and some comments.

Any possible way to do it?

Hi @Mansi thanks for stopping by the community! We don’t have a table option you can add into the forms, but would the matrix question work for you? There have been some creative use cases using this field from other customers like @john.desborough !
