Multiple answer types in 1 question.

I would like to create a form to collect information from clients.  However, I would like one question to have several ways to input - for example

Question 1. please provide the contact information for your business

Short answer: BUSINESS NAME

Short answer: Business Address

Number:  TELEPHONE NUMBER

EMAIL:  EMAIL Address

UPLOAD:  BUSINESS DOCUMENTS

 

Is this possible for one question?  I have several topics like this, that would be best collected all in one instead of separated by several questions.

Hi @Karlee Thanks for stopping by! I would suggest using a Question Group for this, which would group multiple questions together so it doesn’t look like so many questions. Would this work for you?
