Assessment Survey - 30 questions, 10 categories


I’m trying to decide if I can use Typeform (I have a free account pending the answer) or if I need to hire developers to build custom code.   I think Typeform may still be a good alternative to Survey Monkey for another survey I’m building but for this one, I want to present the results to the user on the web page. 

 

This seems simple to achieve but I haven’t found a tool to do it.  Here’s something similar in functionality.

30 questions, rated (low) 1 to 5 (high) could simply use 1 to 5 stars.
10 categories - a score per category showing which areas need the most work.  Not an overall score.

A radial graph would also work overlaid onto a benchmark.
