I am trying to create a question and I want it to look like below:
Rate each statement with 1 - 5:
Statement 1
Statement 2
Statement 3
And then have rating next to each statement.
I hope this makes sense. I want it to be in the same question. Any help would be great

you could use the Matrix question type to create this type of question.
one issue is that you can’t use the responses to a matrix question type for any logic rules at this time.

Thanks John! I appreciate it. I used the matrix but is there no way for me to enable people to add numbers to their answers as opposed to me putting something like this:
I want them to input the number of hours they allocate to each task (1,2,3 etc) but with the matrix, I feel constrained and have to do this
