Is there a form that I can embed on my website that lets you collect name and email in order to download a pdf?

I'd suggest adding a link to your pdf in the submit button and redirect your respondents to this page once they hit this button. Then, they'd be able to download your pdf, and you'd have their name and email.

The redirect on completion is a feature that allows you to send your respondents to a different web page once they're done filling your form. Please, notice that this feature is only available for plans that are plus or above. You can read more about this in this article

Additionaly, you can also edit the URL on the Thank You screen button if you don't want to redirect them straight away. And you can also use hyperlinks as well. The main the requirement for all these is that you host your pdf somewhere which is accessible via a public URL.
