Hey guys,
We have a scoring based form and there are 3 different end screens based on the users score.
On that end screen with the users score there is a button, we want that button to lead to a user info form with name, email, number etc.
What is the best way to do this?

That being said, I would suggest adding in the questions collecting personal information at the end of that form before they hit submit in order to capture that information.
Once they’ve hit the submit button, the only way to capture additional information then would be to redirect them to another form.
