Is it possible to have a multiple choice (Yes or no) but also have a place in the same field where the user can add text for a clarification or elaborate on the yes or no answer?
eg: Do you smoke?
Yes Number of packs per day? __________
No, never smoked
No, but prior smoker
If you previously smoked, what year did you quit? _______________________

It’s currently not possible to collect additional information within the multiple choice question itself, the only option that allows a respondent to write an answer is if you toggle on the Other option.
You could try using logic to branch out to these follow up questions so you can still get those extra details that you need within the form.
