How can I embed this UI page into my theme?

1. How can I embed this UI page into my theme? Do I need to upgrade to the plus version ？
2. Can only the background be changed, or can the click button also be modified?
3. Now click on the answer to automatically jump, can you modify it? Click on the answer, and then click OK, I think this UI interaction method is not very good

  3. It isn’t possible to modify how the form navigates through questions, but I can share this feedback with the product team.
