How to ask two things at the same time in a question

Typeform experts: I want to interview people about specific problems they face in their jobs and ask, from 1-5, a) how severe are these problems and b) how frequently they happen.

Any ideas on how to put those two variants inside a question so people can rate those 2 aspects for the same problem?

Example: “I spend a lot of money to drive traffic to website through marketing”. Please rate the problem:

Frequency: Rate from 1-5

Severity: Rate from 1-5

@lucia_af - here is how i did this type of scenario for a client, who had 3 things they wanted to assess on each question: using the matrix question
