Multiple choice questions

unfortunately Typeform does not allow that to happen on the same page. You would have to use logic branching/jumps to move the person to another question where you can ask for their input.

one of my colleague told me that in the paid version of typeform there was that option

Can you please confirm there is the option to do this type of question?

Example:

5. Did your Manager support you to apply ? 

Yes 
How did he do so? Leave comment

@Eloise_langelaan - that would happen on a second question, in response, probably, to a yes/no response to the question Did your manager support you to apply? if the person clicked yes. 

there is currently no option on the paid plans (I am on the professional plan at the moment) that enables this.
