I would like to know how to be able to add a multiple questions amd have also a text box for them to leave comments to explain their choice.
For example:
Leave comment
5. Did your Manager support you to apply ?
Yes
How did he do so? Leave comment
No
Why not and what did he say? Leave comment

unfortunately Typeform does not allow that to happen on the same page. You would have to use logic branching/jumps to move the person to another question where you can ask for their input.

there is currently no option on the paid plans (I am on the professional plan at the moment) that enables this.
