Hi! Was wondering if it is possible for us to collect First Name, Middle Name and Last Name on one single slide? Saw that its possible for First Name and Last Name under “Contact Info” but not for Middle Name. Anyone can help? 

Hi @ccgoh This is a great question! We don’t currently offer the middle name option in the contact info question, but I can share this feedback to the team. 

In the meantime, you could add three short text questions underneath a question group.
