Hi,
I’d like to create a risk assessment survey (for up to 100 risks) where for each of risk a participant would have to choose a value amongst a list (Iikelihood would have 4 possible value, expressed in %, financial impact would also have 4 values, expressed in $, etc.).
Any idea which form I could leverage?
