I would like the respondents upload videos/photos while answering some questions. Anyone can share the report format that I generated at the end? 
i would like to see if it is suit for my end use. Thank you

Hi @Dobi Happy Fri-yay! Using our file upload question, the respondents can add photos/videos in each of those questions. 

As for the report, anyone can access the link that you generate with the report.
