Can anyone tell me how to add a question that allows people to answer a simple Yes/No, and below including ‘If No, please tell us why’ with free text box to complete.
I can see the Yes/No question option wjhich I am using but I cannot se how to add on the second part to it.
Any help much appreciated.

This is a great question. What I would suggest is to add a short or long text after the yes/no question and then use logic jumps like below to show the “Tell us why” question if they select no.
