Error with SF integration mapping

I am getting this error when I try to activate Form Field Mapping in Salesforce. 

Cannot Activate Mapping

Can't Map Same Salesforce Field To Two Questions: undefined

I have not mapped the same field.

Could I please get tech assistance?
