Ensuring data is held in EEA

We work with a number of UK/EU organisations where we sometimes need to collect special category information. We have been told that we have to ensure that data that is collected is held in the UK/EEA and not be subject to data transfers.

How do we find out which servers the data is held on and if they are on the US servers how can we ‘repatriate’ the data that is held?
