I am trying to add a conditional logic to change the title of a field according to previous fields selection but i am not sure about the feasibility and how can that be done

This is pretty straightforward to do in Typeform. In your third question you can just call the values of the previous questions using @, like below:
The result should be something like this: (if you choose “PCR Rapid test” for Q1 and “4” for Q2)
