Can't get my quiz to show up after using either url share or embed code, everytime I to engage in the quiz this pops up. Any idea why?

I got this the first time i did a form- i had not published it yet.. Have you published the form?

Let us know if you were able to publish the form as suggested by @john.desborough. That's the most common reason when this kind of message appears. :)
