Hi @Kelly H happy wednesday! Did you happen to make any changes to the form? If so, you’ll want to be sure to click the publish button to save those changes so the notifications work. 

Additionally, if that doesn’t work, I’d suggest checking the spam inbox to see if the notifications are there, as they sometimes end up in that inbox.
