Hi @Keith_ Thanks for stopping by the community! We don’t have any other reports of this, and I’m not able to reproduce the issue. Do you mind sharing a video of what happens? This will help us understand the issue further.
Sure, thanks @Liz Here you go…

@Liz Could be a bug fix needed for the new version of Chrome? The version - 96.0.4664.45 that my Android 10 device is using was recently released. I tested rolling it back to an older version - 81.0.4044.138 and the form works as expected when using the autofill fields.

@Liz just to close the loop, it may be that the combination of the Chrome upgrade and my older Galaxy 9+ is the issue. I can’t expect an fix in that scenario unless it’s more widespread. The support team has been quite responsive through this trial and error testing and I appreciate the effort.
Hi @Keith_ Thanks for sending this over and also the update from the support team! It’s definitely possible that it’s the combo of the newer Chrome update. Additionally, it looks like the form is embedded on a website, which could be causing part of the issue based on the embed. Could you try using our standard embed code that we provide to see if this solves the issue?
@liz, it didn’t matter how I tried. It even failed with the demo templates. On the upside, I may be in the market for a new device just in time for Black Friday!
Well, sounds like a good reason to go shopping hehe! If you notice the issue on other devices/browsers, please let me know, @Keith_ !
