Multiple fields in lead capture


 

I would like to create a assessment that has 8 questions and then a lead capture form.

Each of the 8 questions would be presented separately. Based on how their answer all 8 questions, they would have a cumulative score and would be put into 1 of 3 categories (beginner, intermediate, advanced).

I want the responses from each questions as well as the cumulative score to be transferred into our Marketo database. I understand we’ll have to use Zapier.

Once that 8 assessment questions are done, I would like to present a standard lead form that contains 4 fields (first name, last name, email address, company name). This lead info needs to be transferred into Marketo as well. See the attached as an example (we’re not actually having this many fields).

Is this possible? I haven’t been able to figure out how to do this.

Yes, all that you’ve mentioned is possible and it will require calculations and logic jumps. In Typeform:

  • Create the 8 questions
  • Add Email (mark ‘required’ by clicking the pencil icon in the top right corner of that window)
  • Add 3 ‘Short Text’ questions - 1 each for Last Name, First Name and Company Name (mark each ‘required’

Have you identified which question goes to each of the 3 outcomes? Assigning point values to each (Logic → Advanced → Advanced Logic → Logic and Branching) then create logic jumps at the end that direct users to the appropriate category. 

The integration with Zapier to Marketo will prompt you to associate the Typeform field with the proper Marketo field. 

Does that help? I don’t use Marketo so won't be able to show a ‘how to’ video but if you have specific questions, I'd be more than happy to help.

I created a Typeform (in the old version) that does the same thing but associate it with Hubspot.

@djp55 Where you able to put all 4 fields (first name, last name, email address, company name) into your Typeform and display on one page? I’m new to Typeform and that seems to be something we can’t do, unless you can explain to me how.

@JaimeS - this is from another user: unfortunately no, this is not possible at this time. Typeform has been asked to include this on the product feature enhancement list but don’t know where it is in the priority list.

You are correct - it’s not possible to have all these fields on the same page. As @john.desborough mentions, it’s been requested to add this as a feature (I’ve seen the request a few times now).
