Combination of multiple choice and text/data input

Hello, I have two questions:

 

1) Question 1: I would like to create a question with the following structure:

“Identify the requirements of XX in relation to:

  • A: … data/text input
  • B: … data/text input
  • C: … data/text input “

Is it possible to set up this question and make it appear on one page? I’d prefer to have it just on one page as one question rather than splitting it into 3 questions on 3 separate pages.

 

2) Question 2: I would like to create a multiple choice question where the user can then input further data/text information to the options selected, eg:

“Tick the options that apply and describe the strategy used:

  • A - if relevant, select and add description
  • B - if relevant, select and add description
  • C - if relevant, select and add description “

Is it possible? Again, I would like to have this as one question on one page, rather than splitting it into different questions on different pages.

As for the field types you’re describing, I would suggest using a combination of different question types and the Question Group to keep all of the questions housed under one group.
