have you tried embedding your form in amp-iframe? Per AMP docs on amp-iframe it should be used for content like this:
Use
amp-iframefor embedding content into AMP files via iframe. Useful for displaying content otherwise not (yet) supported by AMP.
Please let me know how it works for you, I am curious to see an example if you build one.
