Before our initial phone/Zoom outreach, I have a sizable questionnaire set up for all new customers to complete. There are many various sorts of questions connected to certain goods or services we provide, and I have logic built up to skip through portions of the questionnaire if they do not pertain to the individual doing it. Recently, a few forms have been submitted to us in which we only received a portion of the questionnaire, and it always appeared in a different location (even if there were subsequent needed areas)
