Identify customer response

Can I enter a customer name or a customer number in a feedback form so that I can find the customer's response based on this? I don't want the customer to enter their name or number themselves. How can I enter this?

    do you know how i can modify the response file to include the link/ the hidden file?

    Hi @GaliaZai Welcome to the community! It’s not possible to add the URL to the response, but the hidden field, if collected, should automatically pass into the Results panel.
