Workflow is not recording multiple choice answers.

Hi @Avillar thanks for stopping by! Could you provide a URL of the form you’re referencing or a video of what’s happening? Anything you can share for some context would be really helpful!

The first two snapshots are the multiple choice questions that i want to be recorded from workflow to my email. However once they are completed and submitted i dont get the response as you can see on the third snapshot

1. A screenshot of your notification settings
2. The URL of the form
