Hi
From a fellow community member - on scale questions (score number or stars, for example) you would need to add a separate question. On multiple choice questions, however, you can toggle on the ‘Other’ option by clicking the pencil icon in the top right corner of that question which can be used as a comment field.
Would that work for what you need?

Is it possible e.g. on a Option Scale question to add a commenting text? Asking the the respondent for a comment
