Adding multiple entries for one question


I am looking to give clients the option to add multiple data points for one question 

The question is for a restaurant group and I need to know all their store names and addresses, so for example they would either be asked “how many restaurants do you have” and answer 3 - and be given 3 lines in which to fill out name and address, or have the option to enter one set of information and then add another and another until they have filled all information out for every site.

Hi @ClaireAlvis welcome to the community! Happy to have you here. :grinning: The first step would be to duplicate the questions and add them as many times on your form as you think they might be answered. 

From there, you’ll want to setup logic jumps to show the questions only if they apply using this process here. Would this work for you?
