Employee Responsibility Self Evaluation

I’d like to create a way for my clients’ employees to do the following:

1. List their top responsibilities
2. Rank them based on a love-dislike scale
3. Rank them based on a great-bad skill level scale
4. Compile that data and send to their employer (my client)
