I tested the form, and I didn’t have an issue with restarting once I uploaded images. You can see my test here. 

Is there a certain browser/device you’re noticing the issue on?

You know, one day it works fine, but after 1-2 days it goes with mistake again. I checked it in mobile Safari and actually got same feedback from customers who tried to fill it in. 
I try one again and hope it will work correct.

Actually no, if this will happen again on mobile Safari, I try it on Chrome as well.
