I want to create an information gathering form for clients. I am having trouble creating how a client enters in their medications and how they enter in details of medical treatment doctor/hospital visits over a given period of time. So a client could be taking 1 medication or 20, same variability as to medical treatment.
Paper analogy is a form table. For medications, row named “Medicine Name” with columns for “Prescribing Doctor, “Start Date”, “Dosage” and “Side Effects”. I am having a hard time figuring out ho to set this up. Thank you.
